SUPPORTING DOCUMENTATION
K-12 students not currently attending a Burnaby Public School (under 19 years old)
- Proof of citizenship: for both the parent and the student (Canadian birth certificate, citizenship card, passport, landed immigrant document, notice of decision, permanent resident card)

- STUDENT: Proof of birth date and Canadian documentation showing status in Canada. This could be a Canadian passport, Canadian birth certificate or a Canadian immigration document (PR card, Study or Work Permit, Refugee status)
- FOR PARENT: We require the same information as above for one parent or legal guardian with legal guardianship papers. This is not required for Adult students.
- PROOF OF GUARDIANSHIP: if the parent last name is different than that of the student last name, we require an official document showing you are the parent or guardian (as shown on long birth certificate or other appropriate legal documentation such as landed immigrant paper or guardianship order) **If you are unable to upload, please email to register.online@burnabyschools.ca
REQUIRED PROOF OF RESIDENCE
Two pieces of documentation that confirm parent/guardian residence in British Columbia. Both documents must match the parent listed on your registration whose ID was provided above.
Documentation must include two items, at least one being from Category A.
Category A:
- Proof of ownership of dwelling
- Long-term lease or rental of dwelling
- Legal documents indicating BC residence – Municipal Home tax assessment, etc.
- Parent or Guardian government issued tax statement
Category B:
- Utility bill such as BC hydro, Terasen (FortisBC) Gas
- BC cable bill
- Provincial driver’s license, BCID or BC Services Card (all considered 1 piece)
- Provincial registration of automobile
- Canadian bank account or credit card statement
- Telus land line phone bill (not cell phone)
ACADEMIC HISTORY
Either the latest report card from your current school or a transcript of marks is required.
Adult Students (19 years old or over)
REQUIRED IDENTIFICATION
